What Children are Eligible

All children birth through 18 years of age who are:

  • Enrolled in Medicaid Program
  • Enrolled in Children's Health Insurance Program (CHIP)
  • Without health insurance
  • American Indian/Alaskan Native
  • Underinsured (insurance does not cover immunizations)

  • Diphtheria
  • Tetanus
  • Pertussis
  • Haemophilus influenzae type b
  • Measles
  • Mumps
  • Rubella
  • Hepatitis A
  • Hepatitis B
  • Varicella
  • Pneumococcal Conjugate
  • Polio
  • Influenza
  • Rotavirus
  • Meningococcal Conjugate
  • Human Papillomavirus (HPV)

Provider Enrollment

1. Complete the Provider Profile and Enrollment Form and mail the original to the Utah VFC Program.
2. Complete Storage and Handling Training.
3. Once enrollment is complete, a Utah VFC Program representative will schedule a training session for you and your office staff. The training will include all aspects of the Utah VFC Program.
